Actors miss time on shows for many reasons such as contract disputes, health reasons, maternity leave, personal issues, etc. Here's a few.
Carroll O'Connor missed a few episodes of All In The Family due to a contract dispute.
Suzanne Somers--We all know the deal there.
Bob Newhart was absent from a few episodes of The Bob Newhart Show. I heard his wife was pregnant, and he spent time with her.
Tony Sirico aka Paulie Walnuts missed about 5 episodes of The Sopranos last season due to health reasons.
Gabe Kaplan sat out a number of eps of "Welcome Back Kotter" because of contract disputes.
Katey Segal was MIA on "Married With Children" do to pregnancy complications.
Edward Woodward suffered a heart attack and numerous eps of "The Equalizer" (one of my favorite shows ever) were shot around him while he recovered.
This is just a theory, but I believe the entire cast of the NBC series "Conrad Bloom" walked off the show because it really sucked, and the network solved the problem by never talking about the sitcom again.

Charlotte Rae -- The Facts of Life (I think contract, but not sure)

Kristy McNichol -- Empty Nest (personal troubles)
Lisa Bonet -- The Cosby Show (Different World, then contract I think)
Gary Coleman -- Diff'rent Strokes (health problems)
non-sitcom -- Patrick Duffy -- Dallas (wanted to try new things)

Redd Foxx was missing from the last six episodes of the 3rd season and a few episodes of the 4th season of Sanford and Son due to a contract dispute between Redd and NBC.

Contract disputes:
Carroll O'Connor - All In the Family. They nearly killed him off until Fred Silverman came to the rescue in the 12th hour.
Redd Foxx - Sanford and Son. The show turned into Grady and Son during the tail end of the third season.

Not a sitcom but Carroll O'Connor missed several early episodes of In The Heat Of The Night because of heart problems.
As everybody knows Dick York (Bewitched) left permanently because of back problems.
John Schneider and Tom Wopat(Dukes Of Hazzard) because of contract disputes.
Sara Gilbert & Lecy Goranson ( Roseanne) were absent for a while as they were attending college.

I may be remembering this wrong, but...didn't Valerie Harper have a show named after her, then she left it and her character was killed off? I forget the name of the show.

It was Valerie's Family changed to The Hogan Family after she left. They killed her character off but I can't remember why!:confused:

On Three's COmpany, of course, Suzanne SOmers, but Joyce DeWitt also missed one episode "Stanley's Hotline" anyone know why?

Jane Leeves was absent from a few episodes of "Frasier" because she was pregnent. At first, they hid it in the useaul ways (showing her from the waist up, sitting at a table, carrying something in front of her, etc.) then they had Daphnie go away for a few weeks.
Lorreta Swit missed one episode of "MASH" because she was making a TV movie.

Tracey Gold took a month or so off from Growing Pains to battle (and win) her battle with anorexia.
Jack Soo missed episodes on Barney Miller due to his terminal illness.

Lisa Welchel was not involved in the "First Time" episode of Facts of Life because the storyline was against what she believed in.

Actually, now that I think about it, Loretta Swit missed a few episodes here and there during the first few seasons.
Also on the subject of M*A*S*H; Around season four of five Gary Burghoff began cutting back on the number of episodes he appeared in each season, so there are many episodes during those years he simply wasn't there, and a few times they referred to him as being "in Tokyo on R&R."
In season eight, Radar didn't appear until episode six (Goodbye Radar) when he got his orders to go home. However, they did do a few scenes with him on the phone talking to Klinger during episodes 1-5.
James Best and Ben Jones missed episodes of The Dukes of Hazzard in the second seasons due to backstage conflicts.
The All in the Family actors all missed episodes here and there beginning in 1975.
Peter Boyle missed a couple of episodes of Everybody Loves Raymond due to his heart attack, and Patricia Heaton due to her pregnancy.
Richard Kline and Don Knotts missed episodes of Three's Company.

"Saved by the Bell" - Tiffani-Amber Thiessan (Kelly) and Elizabeth Berkley (Jessie) went off the show for awhile and were replaced by Tori. They later came back at the end for prom and graduation.

All 5 kids have missed episodes here and there, especially Sabrina LeBeauf and Lisa Bonet.

Actually the show went through several name changes. First it was "Valerie," then Valerie Harper was fired from the show and the name was changed to "Valerie's Family: The Hogans," and then it was changed to just "The Hogan Family" :) And do you mean why the character was killed off or how the the character was killed off? If it was why, I think it was just because they probably felt it was appropriate to re-cast the role since the show was named after her and all. And if you meant how Valerie Hogan died, it was mentioned that she died in a car accident (of course this was never shown or anything).

Let's see, off the top of my head:
Danica McKellar, The Wonder Years - wanted more money during the 1991-92 season according to some accounts. Notice she was in only five or six episodes.
Olivia d'Abo, same show - wanted to try new things. Notice she was hardly, if ever, seen during the show's last season from 1992-93.
Robert Reed, The Brady Bunch - was disgusted with some of the scripts, refused to appear in the entire finale.

Cindy Williams, Laverne & Shirley - (to borrow a term from I Love Lucy) enciendo
Michael Learned, The Waltons - wanted to try new things (she wasn't in any of the episodes for the last two seasons IIRC)
